INITIAL CONSULTATION
. Psychotherapy requires a good match of clinician and patient. This initial consultation is your opportunity to meet with Dr. Sivesind and determine if this is a good match for your particular needs.
CANCELLATION/Missed Sessions
Appointments not cancelled 24 hours prior to a missed appointment will result in the full fee being charged with the exception of emergencies. Insurance does not pay for missed sessions, patients are responsible fully for this fee. Voicemail and email are both checked several times per day. I try to reschedule missed appointments within the same week whenever possible.
FEES
All fees are due at the time of service. Credit cards, checks, or cash are all acceptable forms of payment. Dr. Sivesind charges a customary fee of $180 per individual session (cpt code 90834 45 minute session) and $200 for couples’ therapy/family therapy (cpt code 90847). Groups are $80 per session.
Fees for psychological evaluations and psychological testing can vary depending on complexity and can be discussed during initial consultation.
There is some availability for sliding scale fees within certain limits and situations. Alternative fee arrangements can be discussed at the initial consultation if cost is a barrier to you.
Dr Sivesind currently accepts Cigna, Magnacare, and Magellan insurance directly. Those who receive services from Dr. Sivesind that do have insurance with PPO plans will be provided with a bill (sometimes referred to as a ‘superbill’) that can be submitted to the health insurance for reimbursement to you from the insurance company for “out of network” services. If your insurance plan provides for an “out of network” plan and want to work with Dr. Sivesind, you may call your health insurance regarding your coverage for mental health costs and procedures (reimbursement rate, number of allowed visits, authorizations, extensions of services, etc).
